Submitting a Spa Services request form does not guarantee appointment. All appointments must be confirmed by submitting a credit card number. Upon receipt of this form our spa associates will review your request and contact you to confirm the date/time and service and to obtain the necessary credit card information. All appointments are subject to a 24 hour cancellation policy. Any service cancelled with less than a 24 hour notice will be subject to full charge. By providing your credit card number you agree to all of our policies. Please review our Spa Policies for further details.
